General Information:
Account Application
Customers who conduct regular record requests may apply
to have an account with the Crime Information Bureau.
Agencies will be billed monthly for their transactions. If
you wish to apply for an account,
complete
the customer account application form and return it to the
Crime Information Bureau. Upon application approval,
the Crime Information Bureau will notify you of your
account number. You need to use this account number when
submitting record requests and payments. A copy of the
request forms with your account number will be sent to
you. Make copies of these forms and use them to submit
record requests.
Agencies who already have an account with the Crime
Information Bureau do not need to apply for a new account.
They should use their existing account number. To verify
that the information in our database is correct, please
complete the customer account application and write your
account number in the appropriate box.
